Job Description
Job Title: Home Training/Peritoneal Dialysis Nurse
Reports To: Home Modality Manager or Clinic Manager (if no Home Modality Manager)
FLSA Status: Non-Exempt
Summary: Provides general nursing care and education to patients within the Peritoneal Dialysis/Home Training Department.
Essential Duties and Responsibilities:
1. Developing and implementing initial training and retraining programs for patients and their care partners.
2. Coordinating the refinement and approval of patient care protocols.
3. Coordinating the interdisciplinary care team which involves the Physician, Dietitian, Social Worker, Patient, and others.
4. Assessing the patient for selection of modality.
5. Patient and care partner initial and continued education.
6. Monthly in-center or home patient visits.
7. Coordination of patient care.
8. Maintenance of required documentation.
Supervisory Responsibilities:
This position has no supervisory responsibilities.
Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience:
Will have at least 12 months of experience in Clinical Nursing, with previous dialysis experience preferred but not required. Must have a strong interest in patient education programs and demonstrate the ability to actively participate in patient education.
Language Skills:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees.
Mathematical Skills:
Ability to apply concepts of basic math, algebra, and geometry.
Reasoning Ability:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registration:
Must be licensed in the State of Practice. Must maintain a Basic Life Support Certification.
Other Qualifications:
Excellent communication/interpersonal skills, the ability to perform independent nursing with little direct supervision, strong organizational skills, knowledge of teaching/learning concepts and enthusiasm and motivation to inspire patients and their care partners are necessary.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
